Abstract

This prospective study included consecutively patients with lesions in or near the central region during the time period from December 1996 to June 1998. According to the preoperative clinical examination the patients were separated in different groups. Each patient was examined on defined pre- and postoperative dates under the usage of a standard protocol. Intraoperative localization of the sulcus centralis was performed under usage of SEP phase reversal. Subsequently the motor cortex was stimulated by monopolar stimulation (mapping). After limiting different functional areas a constant monitoring was made. The aim of this study is to rule out limitation of intraoperative functional mapping and monitoring by patients with a preoperative paralysis. The identification of the central sulcus by SEP phase reversal was in nearly all patients possible. In two cases a technical malfunction was observed. The following mapping was in two preoprative paretic patients impossible. A technical defect was surely ruled out. Both patients showed pre- and postoperative a severe hemiparesis. Although an intraoperative monitoring of the motor function wasn't practicable the central sulcus could identified in this patients with SEP phase reversal. This results ruled out a serious affection of the motor system as a limitation of intraoperative monopolar stimulation. In summary the achieved data of this study demonstrate that the usage of intraoperative neurophysiologic mapping and monitoring in patients with lesions in or near the central sulcus is a save and helpful method. The combination of different neurophysiologic methods and it's synergies is helpful especially in patients with severe preoperative affection of the motor cortex.
