Abstract In the current study we examined the medium-term results of radiosynoviorthesis in the treatment of inflammatory and osteoarthritic joint diseases.We examined 349 patients in the period from 01/98 thru 12/98. Altogether, 813 joints were treated. In the data acquisition and assessment two standardized scores (clinical and subjective) as well as a new questionnaire developed by us were used. We analyzed the success rate of radiosynoviorthesis in dependence of diagnosis and joint size. Furthermore we investigated other possible factors of influence, e.g. patient age and presence of basic medication. We found a total success rate between 80-88% (dependent on score used) after a follow up period up to 36 months. The chronic inflammatory diseases showed a score dependent treatment success of 82-91%, the osteoarthritis of the middle and large joints - 67-79%, and the small finger joints - 80-92%. The comparison of the results of rheumatic and ostheoarthritic joints did not show essential differences. We compared our results to those of other authors, and we also gave a critical discussion and analysis of 49 journal articles, we consider relevant from our point of view, from a total of 319 related publications we found in the literature. Conclusion: The Radiosynoviorthese is a low-risk, highly effective, economically efficient, ambulatory feasible, and, on the part of the patients, very well accepted therapy option in the treatment of chronic-inflammatory and osteoarthritic joint diseases. It appears very promising for the treatment of diseases of the rheumatic or degenerative joint diseases as well. We received particularly good results with the polyarthrosis of small finger joints. Here the radiosynoviorthesis represents a new therapy option. In justified exceptional cases like chronic synovialitis after implantation of endoprothesis or empyem the radiosynoviorthesis can be tried as ultima ratio before a planned operation
