Abstract

Current techniques for the surgical correction of dysgnathic jaw positions do not regularly include numerical control of the actual relation of the jaw to the skull. This represents a typical task for an intraoperative navigation system. The "Virtual Patient System", a commercially available navigation system designed for applications in maxillofacial surgery and orthodontics, has therefore been evaluated in respect to its practicability. The accuracy of the tracking system integrated in the navigation system was evaluated in laboratory and surgical environments, resulting in considerable enhancements. Subsequently, a pilot study was conducted with 17 patients which underwent a Le Fort I osteotomy during a mono- or bimaxillary osteotomy with or without further segmentation of the maxilla. The navigation system used in this study served to verify the clinical results. Based on the results of this study, a new approach for determining the displacement values in the operation planning has been developed and described in mathematical terms. This approach refers to the occlusal plane and is suitable both for the preoperative determination of the displacement values and for their intraoperative controlling and guidance. The transfer of the planning results to the operation room is thus standardized and nearly independent of user variations. The software implementation of the approach utilizes the results of the navigation system evaluation. It is platform independent and includes a readily learnable, intuitive and clear user interface. The method has been verified firstly by a comparative accuracy study in laboratory and surgical environments and secondly by an intraoperative application study with a group of 12 patients.
