Abstract

The shower PUVA therapy is an innovative form of local PUVA therapy, using trioxalen shower (0,27 mg/l) for 5 minutes, followed by long wave ultraviolet light (UVA) irradiation. The purpose of the present work is the evaluation of the therapeutic effectiveness of shower PUVA in the treatment of psoriasis vulgaris. Here we performed a randomized prospective study with fifty psoriasis vulgaris patients. Shower PUVA (n=27) was compared with the so called Berlin regime (n=23). The Berlin regime is a well known psoriasis treatment, consisting of selective UVB phototherapy in combination with the local treatment liquor carbonis detergens in the evening and anthralin in the morning. The results obtained showed a comparable therapeutic effect for shower PUVA therapy and Berlin regime. Additionally the patients treated with Shower PUVA showed a better compliance and needed a shorter hospitalisation period. Furthermore patients with guttate psoriasis responded better to Shower PUVA than to the Berlin regime. Finally we could detect notable concentrations of trioxalen in the psoriatic scales measured by high performance liquid chromatography. Shower PUVA therapy is an innovative, effective method in the treatment of psoriasis vulgaris and it may represent in the future a suitable therapy option for other dermatoses. The good response to shower PUVA therapy can be explained by the proven penetration of trioxalen into the skin, where UVA irradiation leeds to the formation of photoadducts of trioxalen with DNA and subsequent DNA damage, resulting in an antiproliferative effect.
