Abstract

Preface: The lifespan of the knee and hip endoprosthesis is about 10 to 17 years. For an improvement of the treatment with the endoprosthesis we need to develop a better understanding of the events in the bone-implant-interface. For this reason we analyzed the architecture of the trabecula in the surrounding of the Implants. The question of the anisotropy of the trabecular bone in the surrounding of the hip and knee total prosthesis is not solved yet. The orientation of the trabecula in the surrounding of implants was measured, using a morphometer and a Computer Assisted Design (CAD) Program. Material and methods: 9 different implanted bones were sawed after bioptic explantation or by the autopsy. 46 slides were performed and stained with the ?van Kossa? technique. The same size of the region in the ventral, medial, lateral and dorsal area of microscopic slides were measured. The morphometer was used to obtain the area of trabecular bone on the slide as a parameter of the existing bone quantity. The microscopic images of this regions were exported to a CAD program. The angle between every trabecula and the surface of the implant was determined. The orientation of the trabecula was shared in 3 groups: parallel, oblique or rectangular orientation of the trabecula to the implant. Results: The incidence of the oblique to the surface of the implant lying trabecula was greater than the parallel running trabecula. The rectangular to the implant running trabecula build the smallest group. The distinguishing marks with a significant influence on the orientation of the trabecula were duration of using the implant and loading of the implant. The body side of implantation, and whether cement was used or not, did not have a significant influence. Discussion: In our study the trabecular bone in the area around the implants was anisotrop. The majority of the trabecula had an oblique orientation. We think, that this architecture is the result of the adaptation process of the bone due to implantation.
